- Bay Area House Calls is an innovative primary care health company that provides a true Medical Home Program for the frail adult within their home covering the Southeast Harris county and Galveston county area.
- We have proven in Galveston County to have one of the lowest re-hospitalization rates among our peers with excellent patient outcomes and patient satisfaction surveys.
- Well-established private practice that includes 2 physicians and 10 advanced practice providers that provide care to patients of all ages
- Hiring due to growth and the additional physician will assist coverage in hospitals, nursing homes as well as assist with advanced practitioner oversight
- Spanish speaking required
