- Daily Census: Average of patients per day.

- Clinical Experience: Must provide case logs documenting at least 100 patients seen in the last 24 months representative of the requested scope of practice.

- Certification: Active BLS (AHA or Red Cross) is required.
- Background: Valid DEA registration.
